Cluster B Paradox

If narcissism and psychopathy are the brain's reaction to a traumatic event (self-defense mechanism), why can't they be grateful when they meet someone who loves them and cares for them? They need love because they lacked affection, they were neglected when they were children. Yet, they are cruel and ungrateful when someone loves them? I try to help or understand a narcissist, and I discover they don't want to be helped. They choose to be ungrateful, cold, and emotionally stunted. A few days earlier, they were clingy and begged for my attention.
